FMEA - Failure modes and Effect analysis is a technique to identify the potential problems in system design or process by examining the effects of lower level failures. Based on the result actions are taken for the problems/failures to reduce the re occurrence and reduce the risk if it occurs again.

Failure modes are any potential or actual problem/defect in design or process.

Effect Analysis is study of the effects of the failure/problems.

Steps for FMEA

  1. Identify the effect of each failure  by failure mode analysis and identifiy single failure points that are critical.

  2. Rank each failure and its probability of occurrence

  3. Take actions
